Concentration = vial strength ÷ BAC water. A 10 mg vial with 2 mL of BAC water = 5 mg/mL.
Volume per injection = dose ÷ concentration. A 0.25 mg dose at 5 mg/mL = 0.05 mL.
U-100 units are insulin syringe markings: 100 units = 1 mL. So 0.05 mL = 5 units.
